What is filler?

Filler is a substance injected into different facial areas to add volume, reshape contours, or fill sunken areas.

Fillers generally used in aesthetic treatments are often Hyaluronic Acid (HA), a substance that retains water and can be used to fill tissue in different locations.

Each filler formulation has different properties, including firmness, elasticity, spread, and shape retention. It should therefore be selected to suit the injection site and treatment goal.

Highlights of the filler programme

  • Helps fill sunken areas or areas that have lost volume
  • Helps adjust facial shape and proportions
  • Can refine details in many locations
  • Amount and technique can be selected to suit each individual
  • Adjustments can range from a natural look to greater facial dimension
  • Requires no surgery in suitable cases
  • Hyaluronic Acid fillers can be assessed for dissolution when indicated

What can filler help improve?

Filler can be used to refine facial details in several ways, such as:

Adding volume

Helps fill areas that are sunken, hollow, or lacking volume.

Adjusting facial proportions

Helps balance the relationship between features such as the chin, lips, cheeks, or temples and the facial structure.

Filling deep folds

In certain areas, it can fill folds caused by structure or volume loss.

Adding facial dimension

Can create or refine details in areas that need projection, definition, or continuity of facial contours.

Adjusting asymmetry

In some cases, it can refine differences in volume or shape between the two sides of the face.

Where can filler be injected?

Areas commonly assessed for filler injections include:

Lip filler

Helps add volume, adjust lip proportions, define the lip border and Cupid’s bow, or refine the shape of the lips.

Chin filler

Helps add length or projection, or adjust the chin’s shape to balance the face.

Under-eye filler

Helps fill sunken areas or grooves beneath the eyes and improve the transition between the under-eye area and cheeks in suitable cases.

Temple filler

Helps fill hollow or sunken temples and improve the continuity of the upper facial contour.

Hollow-cheek filler

Helps fill cheeks that lack volume or appear hollow, making the midface look more balanced.

Nasolabial-fold filler

Assessed for filling or adjusting the nasolabial folds, with consideration of the midface structure.

Midface filler

Helps add or restore volume in the front cheek area and can adjust the dimension of the midface.

Each location requires different fillers, techniques, and amounts, so an assessment should be performed every time.

Who is filler suitable for?

The filler programme may be suitable for people who:

  • Want to add volume to sunken or hollow areas
  • Want to adjust facial proportions
  • Want to refine the shape of certain features
  • Have age-related volume loss
  • Have some facial asymmetry
  • Want facial reshaping without surgery in suitable cases
  • Have previously had filler and want an assessment for additional treatment

The doctor will assess the existing structure and individual suitability before planning the injections.

How do filler formulations differ?

Not all fillers have the same properties.

Each formulation may differ in:

  • Gel firmness
  • Elasticity
  • Shape retention
  • Spread
  • Water retention
  • Suitability for different locations

For example, lip fillers may need properties suited to movement, while the chin or certain facial structures may need a filler that provides greater shape support.

There is therefore no single filler formulation suitable for every location and every person.

How many cc of filler are needed?

The amount of filler varies between individuals and depends on several factors, such as:

  • The intended injection location
  • The degree of hollowing or volume desired
  • The existing facial structure
  • The goals of facial reshaping
  • Any previous filler that may remain
  • The properties of the selected filler

Some locations may only need refinement, while some cases require assessment of several areas together.

The number of cc should therefore not be the only factor in deciding.

Can filler lift the face?

The main purpose of filler is to restore volume and adjust facial structure.

In some cases, adding volume in appropriate locations may change facial proportions or the overall appearance, but filler is not directly a lifting treatment.

If there is significant sagging, the doctor may assess whether another programme should be used alongside it.

Can filler make the face look younger?

With age, changes can occur in facial bone, fat, tissue, and skin, making some areas look sunken or lose volume.

Filler placed in appropriate locations can help restore volume or adjust proportions for a more balanced appearance.

However, filler does not address every age-related concern, such as sagging, skin quality, or pigmentation, so each concern should be assessed separately.

Steps in the filler programme

1. Consultation and facial assessment

The doctor assesses facial structure, proportions, and the areas to be adjusted.

2. Assessment of existing filler

If you have had filler before, provide the brand, formulation, amount, and date of treatment, if available.

3. Planning locations and amounts

Locations, amounts, and filler properties are selected to suit the goal.

4. Preparing the area before injection

The area is cleaned and prepared to reduce discomfort according to the clinic’s procedures.

5. Injecting filler according to the plan

The doctor injects filler at the assessed locations and depths.

6. Checking the results after treatment

Balance is assessed and aftercare advice is provided.

How long does filler last?

The duration of results varies between individuals and depends on several factors, such as:

  • Filler brand and formulation
  • Injection location
  • Amount used
  • Injection technique
  • Movement in the area
  • Individual metabolism
  • Aftercare

Hyaluronic Acid filler gradually decreases through the body’s processes over time.

Will there be swelling after filler injections?

Some people may experience swelling, tightness, tenderness, needle marks, or bruising after injection.

The severity and duration depend on the injection site, amount, and individual.

The initial appearance may not be the final shape, so swelling should be allowed to subside and the filler to settle before reassessment.

Preparing for filler injections

Before treatment, tell the doctor about:

  • Underlying medical conditions
  • Any history of drug allergies
  • Current medicines and supplements
  • Previous filler injections
  • The brand and formulation of previous filler, if known
  • Previous filler dissolution
  • Surgery or procedures in the planned injection area
  • Inflammation or infection in the treatment area

Do not stop or change medicines yourself without advice from your treating doctor.

Aftercare following filler injections

Follow the doctor’s and clinic’s advice after treatment. In general:

  • Avoid pressing, massaging, or rubbing the injected area yourself
  • Avoid impacts to the treated area
  • Avoid intense heat initially
  • Keep the injection area clean
  • Take medicines only as directed by the doctor
  • Watch for abnormal symptoms after treatment
  • Attend follow-up appointments as scheduled by the doctor

Is downtime needed after filler?

Generally, you can return to daily activities, but some people may have swelling, redness, needle marks, or bruising.

If you have an important event or activity, allow time for post-treatment symptoms to subside beforehand.

Can more filler be added?

Additional filler can be considered after an assessment, but the existing filler should first be checked for how much remains, where it is located, and the current facial shape.

The same amount is not necessarily needed at every treatment, and the decision should not be based on cc alone.

Can filler be dissolved?

For Hyaluronic Acid filler, the enzyme Hyaluronidase can be used to help dissolve it when the doctor determines that there is an indication.

Dissolution should first be assessed by a doctor because the filler type, location, and problem must be considered.

How do counterfeit and genuine fillers differ?

Choose products with a clear origin and verifiable product information.

Before treatment, you can ask the clinic about:

  • Brand and formulation name
  • Product box
  • Lot number
  • Expiry date
  • Relevant registration information

Price should not be the only consideration because the product and its origin are important for injection safety.

Is filler dangerous?

Filler injection is a medical procedure with potential risks, ranging from common symptoms such as swelling, bruising, or pain to uncommon but important complications such as blood vessel occlusion.

Some facial areas contain important blood vessels and structures, so assessment and injection should be performed by a doctor, and a medical facility should be selected that can monitor symptoms and manage complications.

If symptoms such as unusually severe pain, pale skin or a clear change in skin colour, or visual disturbances occur after injection, contact the clinic or seek medical attention immediately.
